For ACL, the model should be:
quiet knee + full ROM + strong quads/hamstrings + hop symmetry + movement quality + reactive control + psychological readiness + progressive sport exposure. 
For meniscus, the model should be:
all of the above, plus respect for tissue healing, tear type, surgical procedure, rotational/compressive tolerance, and absence of joint-line tenderness/effusion. 
Pain-free does not mean sport-ready.
Symmetry alone does not mean high-performance ready.
And time alone should never be the only clearance criterion.
